Activity description: The goal of the 27th Annual Montana Diabetes Professional Conference, which is co-sponsored by the MT Coordinating Body of ADCES and the MT Diabetes Program (MT DPHHS), to provide healthcare professionals across Montana who serve people with diabetes (PWD) with:

i. Current evidence-based practice guidelines.
ii. Practical ways to implement quality, kind, and person-centered care practices which emphasize health equity, cultural humility, and mental health support.
iii. Resources to function as a member of a care team and to provide care more effectively to patients with diabetes.
iv. Opportunities to integrate technology in clinical care, education and prevention services.
v. Tools and resources to advocate on behalf of PWD.

Target audience: This activity is designed for individuals or groups of diabetes care and education specialists including Nurses, Dietitians, Pharmacists, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.

Your Statement of Credit will be issued upon submission of the evaluation form. Pharmacists must provide their date of birth (MMDD) and NABP ePID to receive credit. Pharmacist learners who fail to provide this information will not receive credit for this activity. Please check your NABP profile within 45 days of this activity to make sure your credits were uploaded. Per the American Academy of Nurse Practitioners Certification Board, nurse practitioners are eligible to claim continuing education contact hours from ANCC
