This workshop equips DSMES Quality Coordinators and program leaders to implement Continuous Quality Improvement in alignment with the National Standards for DSMES. Participants learn how to collect, track, and analyze behavioral and clinical outcome data, translate data into actionable CQI projects, and document progress using audit-ready practices. The session emphasizes practical data workflows, baseline and follow-up tracking, six-month review cycles, and clear alignment between outcomes, improvement goals, and DSMES services. Real-world examples, worksheets, and guided audit exercises support both initial accreditation and ongoing compliance.
Learning Objectives
At the end of this presentation, participants will be able to:
1. Identify and collect at least one behavioral and one clinical or quality-of-life outcome using standardized DSMES data tracking methods.
2. Analyze baseline and follow-up data to define a measurable CQI aim aligned with DSMES services.
3. Develop audit-ready CQI documentation that demonstrates ongoing data review and improvement at a minimum of every six months.
