Description

Diabetes is not a one size fits all health state. Creating a learning environment that promotes optimal education and motivation for the person with diabetes is key to successful implementation. Identifying learning styles and adjusting the education environment based on participant dynamics yields improved health outcomes, increases achievement of patient centered goals and sustains program growth. In this session you will learn how two programs deliver care and education in suburban and rural populations.

Learning Outcome

Attendees will have the skills to create dynamic education programs based on the population, interactive curriculum, class size and differentiating education needs of the virtual learner and the in-person learner.

Learning Objectives

At the end of this presentation, participants will be able to:

Design a DSMT learning format unique to participant dynamics
Use key demographic factors to create educational content that is relevant to the participant population
Develop appropriate learning strategies based on group/individual barriers

This activity is designed for individual or groups of diabetes care and education specialists, including nurses, dietitians, pharmacists, pharmacy technicians, nurse practitioners, clinical nurse specialists, physicians, PAs, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.

If the activity is dietetics-related but not targeted to RDs or DTRs, CPEUs may be claimed which are commensurate with participation in contact hours (One 60 minute hour = 1 CPEU. Certified Diabetes Care and Education Specialists

To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers (www.cbdce.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.

Board Certified Advanced Diabetes Management (BC-ADM)

ADCES is the administering body for the Advanced Diabetes Management credentials. Continuing education programs offered by ADCES can be used toward fulfilling BC-ADM Certification and recertification requirements.

Other Health Professionals

It is the responsibility of each participant to determine if the program meets the criteria for re-licensure or recertification for their discipline.
