Activity description: The overall goal of the "Diabetes Dynamo: Empowering Lives through Prevention & Education" event is to empower participants to incorporate the 2024 Diabetes Standards of Care into their professional practices, using diverse strategies to recognize and address diabetes distress, effectively apply telehealth in rural settings, understand and manage the interplay between diabetes and renal health, implement evidence-based care for diabetes during pregnancy, and address obesity as a comorbidity. These efforts aim to enhance clinical outcomes, increase care accessibility, decrease complications, and reduce disparities for people with diabetes

At the conclusion of this educational activity, participants will be able to:

  1. Identify and implement strategies to recognize and address diabetes distress, thereby enhancing patient coping mechanisms and improving diabetes management outcomes.
  2. Apply telehealth solutions effectively in rural settings to enhance diabetes care accessibility, patient engagement, and health outcomes
  3. Explain the interplay between diabetes and renal health and apply strategies for the early detection, management, and prevention of diabetic nephropathy
  4. Describe evidence-based practices for managing diabetes during pregnancy and implement comprehensive care strategies to optimize maternal health and fetal outcomes
  5. Integrate knowledge and tools to address obesity as a comorbidity in diabetes management, emphasizing comprehensive plans to improve patient outcomes and quality of life

Target audience: This activity is designed for individuals or groups of diabetes care and education specialists including Nurses, Dietitians, Pharmacists, Pharmacy Technicians, Social Workers, PAs, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.

Requirements for successful completion: For successful completion, participants are required to attend the full activity, and complete and submit the program evaluation at the conclusion of the educational event. Your Statement of Credit will be issued upon submission of the evaluation form. Pharmacists must provide their date of birth (MMDD) and NABP ePID to receive credit. Pharmacist learners who fail to provide this information will not receive credit for this activity. Please check your NABP profile within 45 days of this activity to make sure your credits were uploaded. Per the American Academy of Nurse Practitioners Certification Board, nurse practitioners are eligible to claim continuing education contact hours from ANCC

Certified Diabetes Care and Education Specialists: To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers (http://www.cbdce.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.
