This certificate program is a self-study, self-paced program. There are approximately 11 hours of coursework. There is no CE/CME credit for this activity, but learners will earn a certificate of completion upon successful completion of all components.

Intended Audience

This activity is designed for members of the diabetes care team focused on linking individuals to resources, building relationships, and supporting people with, or affected by, or at risk for diabetes and cardiometabolic conditions.

This includes, but is not limited to: community health workers (CHW), health educators/coaches, medical assistants (MA), certified nursing assistants (CNA), licensed practical nurses (LPN), registered nutrition/dietetic technicians (NDTR), military medics and corpsmen, pharmacy assistants/technicians, physical therapy assistants, nutritionists, dental hygienists, emergency medical technicians (EMT), and other similar roles.

Access Period

You have TWO (2) months from the date of your program registration to complete all program activities, submit the final program evaluation, and claim your certificate of program completion.
