Think outside the box to consider creative solutions to diabetes care & education challenges. Presenters will share expertise on combating clinical inertia within professional scope of practice, addressing challenges related to social determinants of health, communicating effectively to evoke behavioral change, educating clients on disaster preparedness, & forging effective community partnerships.

1. Describe within-scope strategies to address clinical inertia
2. Discuss ways to address needs associated with significant social determinants of health (i.e. unhoused/incarcerated)
3. List possible solutions to the challenge of eating healthily on limited income
4. Discuss effective client-educator communication and coaching techniques
5. Summarize disaster preparedness guidance for persons with diabetes
6. Recognize creative DSME-community partnerships that serve CO residents with diabetes
