Fueling a Healthier Future: A Wyoming Focus on Diabetes and Heart Health Virtual Fall Conference

Activity description: Learners will report increased knowledge and/or confidence in: managing diabetes complicated by thyroid disease, applying diabetes technologies such as continuous glucose monitors (CGMs) and insulin pumps, and delivering education effectively in rural Wyoming. Learners will also demonstrate greater awareness of public health programs and resources available to support people with diabetes, leading to more effective referrals and improved patient outcomes.
Target audience: This activity is designed for individuals or groups of diabetes care and education specialists including Nurses, Dietitians, Pharmacists, PAs, Physicians, Social Workers, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.
Requirements for successful completion: For successful completion, participants are required to attend the full activity, and complete and submit the program evaluation at the conclusion of the educational event. Your Statement of Credit will be issued upon submission of the evaluation form. Pharmacists must provide their date of birth (MMDD) and NABP ePID to receive credit. Pharmacist learners who fail to provide this information will not receive credit for this activity. Please check your NABP profile within 45 days of this activity to make sure your credits were uploaded. Per the American Academy of Nurse Practitioners Certification Board, nurse practitioners are eligible to claim continuing education contact hours from ANCC

Analyze and interpret patient-generated data (e.g., from CGM reports, pump downloads) to facilitate shared decision-making and personalize diabetes management strategies.
