Description
This 60 minute session will provide an overview of the diabetes assessment process to aid in diabetes self-management education and support. The assessment process must be tailored, not only specific to the person with diabetes but also for the diabetes care team. Given a multidisciplinary team involved in DSMES with varied experience and resources to enable and structure assessment, this program will help to align on purpose and best practices across care settings and disciplines.

At the end of this presentation, participants will be able to:
--Understand what, why and how to submit annual reports to DEAP, required in order to maintain accreditation.
--Know what data to collect each year to prepare for annual reporting.
--See the online steps for submitting their annual status report (ASR).

Intended Audience
This activity is designed for individual or groups of diabetes care and education specialists, including nurses, dietitians, dietetic technicians, pharmacists, pharmacy technicians, nurse practitioners, clinical nurse specialists, physicians, PAs, social workers, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.
