LilyLink

LilyLink is a digital health platform for managing diabetes during pregnancy.

The app connects with CGMs (Dexcom, Freestyle Libre) or Bluetooth glucometers, and automates fasting and 1hr/2hr postprandial blood sugar logs in accordance with prenatal care guidelines from American College Of Gynecology. In the app, patients can record food, medication, and physical activity, while providers view data and trends in a summary dashboard.

The platform consists of a free mobile app for patients and a connected dashboard for healthcare providers. The platform also provides EHR-integrations and support for remote patient monitoring (RPM).

LilyLink enhances patient care, improves provider efficiency, and ensures secure data sharing, making gestational diabetes management easier and more effective.

Diabetes care and education specialists can help you set priorities and work with you on improving your own management in each of the following areas:

  • Healthy Eating - Consuming a wide variety of nutrient-rich foods for health.
  • Monitoring - Tracking data influencing diabetes outcomes.
  • Reducing Risks - Recognizing risks and adopting behaviors to prevent diabetes complications.

  • To use LilyLink, patients must be given an access code generated by a provider.

Clinic Version Details: Providers can bill newly-enabled RPM codes to cover the cost of the LilyLink platform

Insurance Coverage Details: Most states and both PPO and Medicaid payers cover RPM for high-risk pregnancies with diabetes.
