Pumps, AID Systems & Patch Pumps

All insulins below are U-100 rapid-acting unless noted. Compatibility is device-model-specific — an insulin listed for one device is not automatically listed for another from the same manufacturer.

Rapid-acting insulin compatibility by pump / AID system
DeviceNovoLogFiaspHumalogAdmelogLyumjevApidraKirsty
Tandem t:slim X2 / Control-IQ+
Tandem Mobi1
Medtronic MiniMed 780G
MiniMed Flex
Beta Bionics iLet2!3
Sequel twiist
Omnipod 5
Omnipod DASH4
Omnipod GO
CeQur Simplicity
V-Go

1 Tandem Mobi does not inherit the t:slim X2's Lyumjev clearance — listed for Humalog and NovoLog only.

2 Fiasp is listed for iLet only in the prefilled Fiasp PumpCart cartridge, not from a vial.

3 Kirsty / iLet: named in the iLet User Guide fill instructions but not on the Beta Bionics safety/compatible-drugs page. Confirm against the current User Guide before relying on it.

4 Apidra wear time with Omnipod DASH is up to 48 hours, versus 72 hours for the other listed insulins.

Connected Pens & Caps

Smart pens and pen caps meter insulin differently than pumps. Compatibility is tied to the cartridge or prefilled pen format, not the molecule alone.

Medtronic InPen (also MiniMed Go)5
Bigfoot Unity — White Cap

5 InPen has no compatible 3.0 mL cartridge format for Lyumjev — this is a packaging gap, not a clinical incompatibility.

Bigfoot Unity uses pen-device-specific caps (KwikPen, FlexPen, FlexTouch, SoloStar) and also covers U-200/U-300 concentration pens and unbranded biologic equivalents not shown in this simplified chart — confirm the exact pen on Bigfoot's current compatible-insulins list.

Long-Acting / Basal Insulin

Only one connected device in this reference meters basal insulin.

Before You Prescribe, Fill, or Train

  • Confirm the exact device model. Compatibility is tied to the specific device, not the manufacturer or brand family.
  • Confirm insulin concentration. All devices above are intended for U-100 insulin unless noted. U-200, U-300, and U-500 insulins should not be substituted unless the device labeling specifically allows it.
  • Confirm the insulin name and formulation. Some devices list specific branded products, biosimilars, or cartridge formats — not all insulin aspart or insulin lispro products are automatically interchangeable for device use.
  • Watch for biosimilar substitution. Kirsty is the only interchangeable rapid-acting biosimilar in the U.S. — a pharmacist may substitute it for NovoLog without prescriber sign-off, subject to state law. A device's product list not naming a biosimilar doesn't automatically make it off-label; check the device User Guide and the insulin's prescribing information (section 2.2).
  • Use shared decision-making. Insulin and device choices should weigh clinical needs, safety, cost and coverage, access, dexterity, vision, technology comfort, and the preferences of the person using the device.
Device labeling and insulin compatibility change over time. This is a point-in-time reference, not clinical decision support. Always verify against the current instructions for use, safety information, or prescribing information before making clinical decisions.
