International Consensus Brings Clarity to Diabetes Technology Use in Pregnancy

Pregnancy significantly intensifies the complexity of diabetes management for both individuals living with diabetes and the clinicians who support them. While diabetes technologies such as continuous glucose monitoring (CGM) and automated insulin delivery (AID) systems have advanced rapidly, guidance on how to use these tools safely and effectively during pregnancy has not always kept pace. A newly published International Consensus Statement on Diabetes Technology in Pregnancy helps close this gap, offering timely, expert-driven recommendations to support clinical care.

What Is the Consensus Statement?

The consensus statement was developed by an international, multidisciplinary group of experts in endocrinology, obstetrics, maternal–fetal medicine, and primary care. Endorsed and supported by 25 international organizations, including the Association of Diabetes Care and Education Specialists (ADCES), the statement offers 14 expert-backed recommendations focused on the use of CGM and AID for pregnant women with:

  • Type 1 diabetes
  • Type 2 diabetes
  • Gestational diabetes

The guidance focuses on how diabetes technology can be used to reduce management burden while improving maternal and neonatal outcomes.

Why This Guidance Matters Now

In recent years, multiple studies have demonstrated that CGM and AID use during pregnancy can lead to clinically meaningful improvements. Evidence suggests benefits including:

  • Increased time in the pregnancy-specific glucose target range
  • Reduced risk of large-for-gestational-age infants
  • Improved glycemic stability during a period of rapidly changing insulin needs

Despite these findings, many clinicians have had limited guidance on how to implement evolving technologies within the unique physiological demands of pregnancy. Existing clinical guidelines have not always reflected the latest evidence or real-world experience, leaving care teams to navigate device use, glucose targets, and patient education without consistent direction.

This consensus statement helps bridge that gap by translating emerging evidence and expert clinical experience into practical recommendations that diabetes care teams can apply across settings. Importantly, it acknowledges pregnancy-related insulin resistance, fluctuating insulin requirements, and the added cognitive and emotional burden placed on pregnant individuals.

DiaTribe and the Time in Range Coalition

The consensus was convened by diaTribe, a long-standing leader in diabetes education and advocacy and host of the Time in Range (TIR) Coalition. TIR is especially relevant during pregnancy, when tighter glucose targets are associated with improved maternal and neonatal outcomes.

By centering CGM metrics and pregnancy-specific targets, the guidance supports care teams in moving beyond A1C alone and toward more actionable, day-to-day measures of glycemic control. This aligns with growing recognition that TIR can enhance clinical conversations, support shared decision-making, and improve self-management education.

Why ADCES Signed On

The Association of Diabetes Care & Education Specialists (ADCES) has been a member of the TIR Coalition since its inception, reflecting its commitment to educating both health care professionals and people with diabetes about how TIR can enhance self-management. Pregnancy introduces challenges beyond routine diabetes care, and diabetes care and education specialists play a critical role in supporting sustained technology use, data interpretation, and ongoing adjustment during this period.

ADCES supports this consensus as an important step toward making diabetes technologies more accessible, better adopted, and more effective during pregnancy.
