DEAP Accredited Programs

Annual Status Reporting

Once we accredit your DEAP program, you’re committed to maintaining the national standards for Diabetes Self-Management Education and Support (DSMES) on an ongoing basis. To maintain accreditation, the required DEAP submissions must include a completed CQI project and aggregate program data showing at least one DSMES intervention (1 person) each year. 

DEAP Renewal Instructions

Accreditation is awarded in 4-year terms. To avoid a gap in reimbursement, submit your DEAP renewal application at least 4-6 weeks before your accreditation expires. You can find your accreditation dates on your DEAP certificate and in your DEAP Dashboard.

The renewal application is an abbreviated version of the initial application. You can access it when it’s time to renew in your DEAP Dashboard.

Change of Status

When your accredited program has a significant change in status, you must notify ADCES® within 30 days by email at [email protected] or via the DEAP Dashboard. Changes that need to be reported include but are not limited to the following.

  • Adding, removing or changing site(s) 
  • Having a change of quality coordinator (QC)
  • Change in sponsor, sponsoring organization, or sponsor's contact information
  • Program name change. Administrative fees may apply (see below).
  • Other significant program changes, such as closing, mergers or suspension of the program, etc.
  • Adding and removing team members

For new team members involved in DSMES delivery, you must add them to your DEAP Dashboard within 30 days, including their name, email address, phone number and role. Be sure to maintain their current licensure/registration and certificates showing 15 CEU hours or a current CDCES or BC-ADM® certificate in your accreditation binder.

 

Name Change: Changing the name of main or branch locations will incur a $100 fee to receive a new certificate.

New Branch location: Adding a new location that will require certificates will incur a $100 fee for each certificate.

You may pay by check or credit card. If paying by check, please note this on your DEAP Dashboard. Be sure to write “DEAP” and your program ID number (located on your certificate) in the memo, and mail it by a tracked method to:
