Description

This webinar presents evidence-based strategies for preventing Type 2 Diabetes using a lifestyle medicine framework. Participants will review the pathophysiology of insulin resistance and learn how to translate the six pillars of lifestyle medicine into practice. The session also reviews long-term data demonstrating risk reduction from intensive lifestyle interventions and contrasts this comprehensive therapeutic model with conventional programs with the National Diabetes Prevention Program (DPP).

This webinar counts toward the CDC requirement of 2 hours of continuing education for lifestyle coaches.

Learning Outcome

At the conclusion of this activity, participants will be able to:  
• Describe Lifestyle Medicine  
• Accurately differentiate Lifestyle Medicine from the National DPP using at least three evidence-based distinctions  
• Develop a patient-centered prevention plan incorporating all six pillars of Lifestyle Medicine  
• Identify at least two measurable metabolic outcomes (beyond weight loss) to monitor clinical effectiveness in patients with prediabetes.

Learning Objectives

At the end of this presentation, participants will be able to:    
• Discuss the pathophysiologic drivers of Type 2 Diabetes development.   
• Differentiate the Lifestyle Medicine approach from the National DPP.   
• Interpret key findings from landmark prevention trials and long-term follow-up studies supporting intensive lifestyle intervention.  
• Design a prevention plan incorporating all six pillars of Lifestyle Medicine.   
• Apply an interprofessional care model to deliver sustainable, equity-centered diabetes prevention strategy.   
• Identify at least 2 measurable clinical outcomes beyond weight loss.

Sorogi, Inc.

Dr. Vivian Ayuk is the founder of Sorogi Inc. She is also an Alumna of the University of Minnesota College of Pharmacy with over 20 years of experience in chronic disease management. 

She has led multiple successful pilot studies incorporating technology tools in diabetes management. As Program Director at Sorogi, she has partnered with several Federally Qualified Health Centers (FQHCs), Managed Care Organizations (MCOs), and private practices in Washington, DC, to implement community-based programs for diabetes and blood pressure management, as well as improve access to technology tools, including continuous glucose monitoring (CGM) devices. Currently, she is leading the DC Health Chronic Disease Division Diabetes Capacity Building project to increase the number of community pharmacies capable of offering Diabetes Self-Management Education and Support (DSMES) services to residents in underserved neighborhoods. 

In addition to her community work, she is a member of the Adjunct Clinical Faculty at the Notre Dame of Maryland School of Pharmacy and the Association of Diabetes Care and Education Specialists (ADCES). She serves as an advisor to DiabetesMine and DiabetesToolKit.

Certified Diabetes Care and Education Specialists

To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers (www.cbdce.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.

Board Certified Advanced Diabetes Management (BC-ADM)

CBDCE is the administering body for the Advanced Diabetes Management credential. Continuing education programs offered by ADCES can be used toward fulfilling BC-ADM certification and recertification requirements.
