The twiist™ AID System is designed for individuals with type 1 diabetes ages 6 and older. It automates insulin delivery based on real-time continuous glucose monitoring (CGM) data, predicted glucose trends, and user-defined parameters. The system is intended to support flexible, personalized therapy with simplified settings designed to accommodate a range of individual lifestyles and multiple wear options for added discretion and comfort. The twiist AID System allows for full iPhone control and supports bolus delivery from Apple Watch.

At the core of the system is twiist Loop, an adaptive algorithm derived from the original open-source Loop platform created by and for people living with diabetes. twiist Loop adjusts basal insulin delivery in real time based on CGM data, predicted glucose values, and individualized targets. The algorithm offers a high degree of customization, including the ability to set a broad glucose correction range (87–180 mg/dL) and modify meal entries retroactively.

The system incorporates iiSure™ Technology, which directly measures the volume of each insulin micro-dose as it is delivered. This real-time verification enables earlier detection and alert of insulin delivery issues including occlusions.

Infusion Set:
The primary infusion set for the twiist system is the Smiths Medical Cleo 90 Infusion Set. Please refer to the twiist user guide for a detailed list of compatible infusion sets.
Basal Range:
  • Basal rates can be entered with a minimum value of 0.00 units per hour and a maximum value of 30 units per hour and can be edited in increments of 0.05 units per hour. The minimum basal rate of delivery is 0.1 units per hour.
  • Basal rates may be set in 30 minute intervals throughout the day.
Bolus Range:
Programmable from 0.05 units in 0.01 unit increments; maximum is 25 units
Transmitter Range:
The maximum distance for communication between the iPhone and the twiist pump is 33 feet or 10 meters.

    The twiist™ Automated Insulin Delivery (AID) System powered by Tidepool includes three components:
    • the twiist™ alternate controller enabled (ACE) insulin infusion pump;
    • the twiist™ Loop interoperable automated glycemic controller (iAGC) algorithm; and
    • a compatible integrated continuous glucose monitor (iCGM).
    Indications for Use
    • The twiist System is intended for the subcutaneous delivery of insulin, at set and variable rates, for the management of diabetes mellitus in persons requiring insulin, ages six and above. The pump is able to reliably and securely communicate with compatible, digitally connected devices, including automated insulin dosing software, to receive, execute, and confirm commands from these devices. The twiist system is intended for single patient, home use and requires a prescription.
    • The twiist Loop algorithm is intended for use with compatible integrated continuous glucose monitors (iCGMs) and the twiist alternate controller enabled (ACE) insulin infusion pump to automatically increase, decrease, and suspend delivery of basal insulin based on iCGM readings and predicted glucose values. It can also recommend, and with the user's confirmation, control the delivery of correction boluses when glucose values are predicted to exceed user configurable thresholds. The twiist Loop algorithm is intended for the management of Type 1 diabetes mellitus in persons six years of age and greater. The twiist Loop is intended for single patient use and requires a prescription.
    • The twiist Automated Insulin Delivery System includes a simple bolus calculator available when Loop is off. It is indicated for use for aiding the user in determining the bolus insulin dosage for management of diabetes mellitus based on consumed carbohydrates, operator entered blood glucose, insulin sensitivity, insulin to carbohydrate ratio, target glucose values, and current insulin on board.
    Contraindications
    The twiist automated insulin delivery system is not intended for anyone unable or unwilling to:
    • Test blood glucose (BG) levels as recommended by their healthcare provider.
    • Maintain sufficient diabetes selfcare skills.
    • See their healthcare provider regularly.
    • Demonstrate adequate carbohydrate-counting skills (preferred, not required).
    The user must not:
    • Have difficulty seeing.
    • Have difficulty hearing.
    • Have deficient cognitive capabilities.
    • Have physical impairments which would make operating the pump or iPhone difficult.
