Diabetes Self-Management Education and Support (DSMES) is structured around seven diabetes self-care behaviors, known as the ADCES7 Self-Care Behaviors®. Diabetes care and education specialists can help you set priorities and work with you on improving your own management in each of the following areas:
Mobile’s can access the Enhance-d App with Operating Systems which are either;
Glucose related (also Insulin and carbohydrate information)
Designed and built under the WCAG Standards for Contrast and Readability
Requires Computer or Tablet for Dashboard Access
Mobile’s can access the Enhance-d App with Operating Systems which are either;
Patients require the use of a continuous glucose monitor
Some features benefit from the use of a wearable fitness tracker (e.g. sleep tracking)
Best suited to people who track daily activity via wearables or mobile apps such as Apple Health, Garmin, Strava etc. and use a CMG (continuous glucose monitor).
Patients can share their patient ID via email to clinicians to be added to a clinic’s dashboard account or can self-pair to a clinic inside the Enhance-d Mobile App by scanning a clinic’s QR code on Enhance-d posters/flyers which are available to download & print via the dashboard.
