These Practice Pearls are based on the experience of diabetes care and education specialists and other healthcare professionals. They are intended to support device training and troubleshooting, but should not replace manufacturer instructions for use or individualized clinical judgment.
Thank you to Leah Lambert, RN, BSN, CDCES, CPT for these practice pearls.

    • When patients are unsure whether they recently injected, the dose-memory function can serve as a helpful reference. Reassure patients that the memory is an informational tool for peace of mind, but it should complement, not replace, their structured logging and clinical judgment.
    • NovoPen Echo delivers 0.5 to 30 units in 0.5-unit increments, providing finer dose adjustments for people who require small insulin doses. This feature is particularly valuable for pediatric patients and insulin-sensitive individuals who rely on tight fine-tuning to manage daily glycemic variations.
    • Educate patients to keep the needle in the skin after pressing the dose button and count slowly to 6 once the dose counter reaches 0 before removing the needle.  Removing the needle too soon can result in droplet pooling on the skin and incomplete dose delivery.
    • Use a new needle and perform the 2-unit insulin-flow test before every injection, not only after changing cartridges. Continue until insulin squirts from the needle tip to ensure no air gaps remain in the system.
    • Avoid alcohol, chlorine, iodine, and other harsh cleaning agents on the pen, as chemical exposure can crack or damage the plastic cartridge holder. Clean the exterior according to the manufacturer's instructions using a moistened cloth and an appropriate mild detergent.
    • If an injection is interrupted or the patient is uncertain how much insulin was delivered, instruct them not to assume that the dose-memory display can determine the exact amount received. Follow the manufacturer's instructions and use glucose monitoring or clinical guidance to determine the next step rather than automatically repeating a full dose.
