Building Your Diabetes Prevention Program

In order to obtain the 6 hours of continuing education credit, participants must attend the entire training on both days.

Program Overview

Building Your Diabetes Prevention Program equips participants with information, tools, and guidance to build a strong and sustainable prevention program. Designed specifically for program/quality coordinators, this workshop provides an overview of CDC's Diabetes Prevention and Recognition Program Standards, Medicare DPP supplier requirements, and successful strategies to enroll, engage, and retain participants; build referral relationships with healthcare providers; and maximize reimbursement from public and commercial payers. Through this workshop, National DPP program/quality coordinators can learn new approaches to building an interprofessional team and developing an integrated approach to diabetes care and community health services.

Learning Outcome

The diabetes prevention program coordinator will be able to work with the whole care team to disseminate the knowledge gained to advance diabetes prevention program sustainability to achieve full CDC recognition, become Medicare DPP suppliers, and prevent/delay type 2 diabetes for people with prediabetes.

Learning Objectives

At the end of this training, participants will be able to: 

-- Define the National Diabetes Prevention Program 
-- Apply best practices from Diabetes Self-Management Education and Support Programs (DSMES) to the National DPP 
-- Describe the current CDC Diabetes Prevention and Recognition Program (DPRP) Standards 
-- Manage challenges to achieving CDC preliminary, full, and full plus recognition 
-- Describe the Medicare Diabetes Prevention Program (MDPP) requirements 
-- Outline how to maximize reimbursement opportunities through employers, grant-makers, and Medicare 
-- Discuss considerations in the sustainability of a prevention program, assessing readiness to build a successful, sustainable DPP with a positive return on investment (ROI)

Elva Hooker is the Clinical Administrative Director for the St. Vincent de Paul Virginia G. Piper Medical Clinic. For over 15 years she has provided clinical administrative and operations management, patient care (individual consultation) and group sessions for diabetes management and prevention. She coordinated the accreditation and successful implementation of diabetes management and prevention sessions through ADCES DEAP and CDC DPRP programs at St. Vincent de Paul. Elva has been providing DPP Lifestyle Coach Trainings with ADCES since 2018.

Association of Social Work Boards (ASWB)  As a Jointly Accredited Organization, the Association of Diabetes Care & Education Specialists is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 6.0 general continuing education credit(s).

To receive a Statement of Credit, you must participate in all identified components of the activity and complete an evaluation.

Certified Diabetes Care and Education Specialists

To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers (www.cbdce.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.

Board Certified Advanced Diabetes Management (BC-ADM)

CBDCE is the administering body for the Advanced Diabetes Management credentials. Continuing education programs offered by ADCES can be used toward fulfilling BC-ADM Certification and recertification requirements.

Other Health Professionals

It is the responsibility of each participant to determine if the program meets the criteria for re-licensure or recertification for their discipline.
