FreeStyle Libre 3 System & Hospital Discharge Program for Transition of Care

On behalf of Abbott, the makers of the FreeStyle Libre 3 system, we would like to welcome you to Dr. John Gilbert’s presentation on an overview of the FreeStyle Libre 3 system and the Hospital Discharge Program (HDP), along with the benefits of starting the appropriate hospitalized patients with diabetes at discharge on a continuous glucose monitor (CGM) for a pathway to the continuation of care.  

Dr. John Michael Gilbert, Jr. is an endocrinologist affiliated with St. Jude Medical Center since 1991. He has served in the role of Director of Endocrine Clinical Trials since 2000, Physician Champion to the Diabetes Steering Committee, and is an active participant in improving inpatient and outpatient Endocrine and Diabetes care in St. Joseph and Providence Healthcare System.

With Dr. Gilbert’s clinical expertise in the endocrinology space, he will provide an in-depth discussion on the FreeStyle Libre 3 system, highlight clinical features of FreeStyle Libre 3 system, and specifically how he has successfully implemented it in his healthcare system through Abbott’s HDP program of a patient starter kit of educational resources to set discharged patients up for success with CGM.
