Effective dates for CE credit: 6/15/2023 – 6/15/2025
This webinar will cover what Quality Coordinators are expected to prepare annually to maintain accreditation. The session will cover data requirements for all programs and share best practices for programs of any size, capacity and technology support (or lack thereof).
Participants will know what data they need to collect on an individual and group level for their annual status report; better understand how to set reasonable goals for data collection that are manageable given the support they have access to; and be able to identify key people within or outside of their DSMES that can provide support.
At the end of this presentation, participants will be able to:
-- State what data needs to be collected on an individual and group level for an Annual Status Report
-- Create reasonable goals for data collection that are manageable given the amount of support available
-- Identify key people within or outside of the DSMES service that can provide support
