This webinar will provide DSMES providers with strategies for demonstrating outcomes and value of their DSMES program.
DSMES providers will be able to use population health strategies and population-based performance measures to better support their DSMES programs.
At the end of this presentation, participants will be able to:
-- Identify and create the value of the DCES/DSMES beyond reimbursement
-- Describe how population health and population-based performance measures link to DSMES and the work of DCES
-- Illustrate how to implement population health strategies to support and meet organizational goals
-- Identify how to articulate the value of DSMES to their organization

ADCES Webinars are designed for individual or groups of diabetes care and education specialists, including nurses, dietitians, dietetic technicians, pharmacists, pharmacy technicians, nurse practitioners, clinical nurse specialists, physicians, PAs, social workers, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.
